Introduction

These terms and conditions outline the policies regarding bookings, attendance, no-shows, and late cancellations for nutrition and dietetics services provided by Eat2Wellness.

By booking our services, you agree to comply with these terms.

2.

Bookings and Booking Confirmation

2.1

All bookings must be confirmed by Eat2Wellness via telephone or email.

2.2

The standard consultation fee is $190. You acknowledge that if you have a Medicare care plan from a General Practitioner, there will be a Medicare rebate of $58 leaving you with an out-of-pocket cost of $132 per consultation.

2.3

If you do not have a Medicare care plan, you will be responsible for the entirety of the standard consultation fee. The standard consultation fee is negotiable for concession card holders and at the discretion of Eat2Wellness.

2.4

You agree to provide Eat2Wellness with your credit card details to store on your file for the purpose of payment of standard consultation fee.

2.5

Eat2Wellness will securely store and manage all credit card details.

3.

Attendance

3.1

Clients are expected to arrive on time for their scheduled appointments.

3.2

If you anticipate being late, please notify us as soon as possible. We will do our best to accommodate you, but extended delays may result in a shortened session.

4.

No-Shows

4.1

A no-show is defined as a client failing to attend a scheduled appointment without prior notification.

4.2

In the event of a no-show, you may be charged a no-show fee of $190. Should you have a Medicare care plan from a General Practitioner, this will not be redeemable against no-show fees.

5.

Cancellations and Late Cancellations

5.1

Cancellations must be made at least 24 hours before the scheduled appointment. If you cancel at least 24 hours before the scheduled appointment, you will have the option to reschedule the appointment.

5.2

Late cancellations made within 24 hours of the appointment will result in payment of a cancellation fee of $65.

5.3

In the event of a cancellation by Eat2Wellness, you will have the option to reschedule the appointment.

6.

Amendments to Terms

6.1

Eat2Wellness reserves the right to amend these terms and conditions at any time. Clients will be notified of any changes in writing.

6.2

Payment of any no-show or cancellation fees are subject to the discretion of Eat2Wellness.
